The following is a list of items are asked in our scholarship application:

• Highschool weight GPA, Class Rank, Class Size
• School Activities - Provide the year (s) participated and the approximate hours per week.
• Community Activities - Provide the year (s) participated and the approximate hours per week.
• Honors / Awards/ Leadership Positions - Provide the year(s)
• Interests & Hobbies
• College Fraternity and Sorority Affliation of Relatives (Include name, relationship, and sorority or fraternity affliation of each)
• Employment (If you are employed, note where you work and the hours you work per week)
• State how this scholarship would benefit you
• State to which colleges and universities that you've applied to for admission (If you have been admitted then note which ones have accepted you or the one you plan to attend)
